Fill out the top part of the application and sign your name. Make sure to get a parent/guardian signature.

• On the back, list your program choices in order of preference. For example, if Restaurant Services is your first choice, mark it with the number 1;
• Then mark your second and third choices. If you are interested in Auto Technology or Cosmetology, please mark your preference for location.
• Have a parent/guardian fill out the Emergency Medical Information.
• Take the completed application to your guidance counselor.
